BREAST AUGMENTATION

Breast augmentation (also referred to as chest feminization or top surgery for trans women) is an excellent option for trans women and nonbinary individuals who are seeking a more feminine-looking upper body.

GENDER-AFFIRMING BREAST AUGMENTATION | CHEST FEMINIZATION | TOP SURGERY FOR TRANS WOMEN

Gender-affirming breast augmentation is a type of surgery that helps create a fuller, more traditionally feminine chest. It’s often chosen by transfeminine and nonbinary people who want their chest to better reflect their identity. For many, this surgery is not just about appearance, it’s about feeling more at home in their body.

Chest feminization is a powerful step toward feeling seen, safe, and confident for many in the transfeminine and nonbinary community. It can reduce gender dysphoria, boost self-esteem, and help people feel more at ease in social situations and in their daily lives. [1]

Surgery like this can also help people express their true selves more freely, both physically and emotionally. Having a chest that matches your gender identity can bring a sense of peace and confidence.

While every person’s journey is different, gender-affirming breast augmentation can be a meaningful and empowering choice for those who want their body to better reflect their gender identity.

WHAT IS GENDER-AFFIRMING BREAST AUGMENTATION?

Gender-affirming breast augmentation is a surgery that uses implants to create a fuller chest for people who want a more traditionally feminine shape. While the surgical steps may be similar to cosmetic breast augmentation, the reasons for choosing the surgery are often very different.

Cosmetic breast augmentation is usually done to enhance breast size or shape for aesthetic reasons. In contrast, gender-affirming breast augmentation is part of a person’s transition journey, a way to help their body match their gender identity. [2]

People who may seek this surgery include:

1). Transfeminine individuals (such as transgender women)

2). Nonbinary individuals who want a more feminine chest

One of the most important parts of this decision is body autonomy. Everyone deserves the right to make choices about their own body. Whether someone wants breast augmentation, chooses not to have surgery, or explores other ways to affirm their gender, the decision should come from a place of self-trust and empowerment.

In short, gender-affirming breast augmentation isn’t just a medical procedure, it’s often a meaningful and powerful way for someone to feel more like themselves.

HORMONE THERAPY AND BREAST DEVELOPMENT

For many transfeminine and nonbinary people, hormone replacement therapy (sometimes called HRT or estrogen therapy) is a key part of the gender-affirming journey. One of the effects of estrogen is breast development, which can help create a more traditionally feminine chest over time. [3]

However, not everyone sees the same results. Some people may only grow small breast tissue even after being on estrogen for a year or more. Genetics, age, and the duration of hormone use can all impact the extent of breast growth.

Because of this, many people choose to have gender-affirming breast augmentation surgery even after starting hormones. Surgery allows for more control over breast size and shape, especially for those who want a fuller chest or who didn’t get the results they hoped for with hormones alone. [4]

Most surgeons recommend waiting at least 12 to 24 months after starting estrogen before having breast augmentation. This gives your body time to go through natural hormone changes and for breast tissue to grow as much as it can on its own. Waiting also helps the surgeon better plan your procedure, because the natural development can affect implant choice and placement. [5]

Overall, hormone therapy can help breast growth, but it might not always give people the results they want. Breast augmentation is a safe and affirming next step for those who want a more defined chest that matches how they feel inside.

IMPLANT OPTIONS AND SURGICAL TECHNIQUES

If you’re thinking about gender-affirming breast augmentation, one of the important steps is deciding what kind of implants and surgical technique will work best for your body and goals. Your surgeon will help guide you through these choices, but here’s a basic overview to help you feel informed and empowered.

Types of Implants: Saline vs. Silicone

There are two main types of breast implants: saline and silicone.

  • Saline implants are filled with sterile salt water. They are pre-filled or are inserted empty and then filled after being placed in the chest. If they ever leak, the body safely absorbs the saline. However, some people feel that saline implants don’t feel as natural as silicone. [6]
  • Silicone implants are pre-filled with a soft silicone gel that closely mimics natural breast tissue. Many people feel that these implants look and feel more like real breasts. However, if they rupture, the leak may not be noticeable without imaging tests, such as an MRI. [7]

Both options are considered safe, and your surgeon can help you decide which is better suited to your body and your preferences.

Placement Options: Under vs. Over the Muscle

The implant can be placed in two main locations:

  • Under the muscle (submuscular): The implant goes beneath the chest muscle. This option often appears more natural, especially for people with little natural breast tissue. It can also reduce the risk of visible rippling in the implant. [8]
  • Over the muscle (subglandular): The implant is placed just under the breast tissue but above the chest muscle. Recovery may be faster, but there could be more visible implant edges if you have very little natural tissue. [9]

Your anatomy, amount of breast tissue, and your aesthetic goals will influence the best placement choice.

Incision Locations: Inframammary vs. Periareolar

Surgeons can also use different incision sites to insert the implant:

  • Inframammary incision: This is a small cut made in the fold underneath the breast. It’s the most common method, and gives the surgeon a lot of control. The scar is usually well-hidden under the breast. [10]
  • Periareolar incision: This incision is made around the edge of the areola (the darker skin around the nipple). [10] It may be less noticeable, but it’s usually only recommended if the areola is large enough to safely make the incision.

PREPARING FOR SURGERY

Getting ready for gender-affirming breast augmentation involves more than just picking a date. It’s about feeling informed, confident, and supported, both physically and emotionally, through the whole process. Here’s what to expect as you prepare:

What to Expect During Your Consultation

Your first consultation is a big step. During this appointment, you’ll talk with your surgeon about your goals, your body, and your medical history. The surgeon will explain what’s possible and help you understand what results you can expect.

They’ll likely examine your chest, talk about implant types, placement options, and incision techniques, and may take some photos for planning. This is also a chance to build trust with your care team and ask all your questions. [11]

Questions to Ask Your Surgeon

It’s helpful to bring a list of questions with you. Here are a few good ones:

1). What type of implants do you recommend for me?

2). Where will the incisions be, and what kind of scars can I expect?

3). What’s the recovery process like?

4). How many gender-affirming surgeries have you done?

5). Can I see before-and-after photos of your past patients?

Don’t be afraid to speak up. Your voice matters. A good surgeon will answer honestly and support your choices.

Medical and Emotional Readiness

Before surgery, you’ll need to be medically cleared. This may include lab work, a physical exam, or a check of any existing health conditions. In some cases, you may also need to stop certain medications or adjust your hormone therapy briefly; your care team will guide you.

Depending on where you live or what provider you choose, you might also need a mental health letter confirming that you’re ready and that this surgery is part of your gender-affirming care. This is often based on the WPATH (World Professional Association for Transgender Health) guidelines. [12]

Some people feel excited before surgery, while others experience nervousness or emotional distress. All of these feelings are valid. It’s important to check in with yourself and lean on support systems, like friends, therapists, or community groups during this time.

CHOOSING A SURGEON

Choosing the right surgeon for your gender-affirming breast augmentation is one of the most important parts of your journey. You deserve to feel seen, respected, and safe every step of the way. Here’s what to look for when picking a surgical provider.

What to Look For in a Gender-Affirming Surgical Provider

Look for a surgeon who has experience performing gender-affirming surgeries, not just general cosmetic procedures. A provider who understands the specific goals and needs of trans and nonbinary patients can offer better care and outcomes. Align Surgical specializes in gender-affirming procedures and works closely with each patient to create a personalized plan.

You should also check that your surgeon is board-certified or in the process of certification and trained in the techniques you’re interested in. A provider with this background can ensure your safety and satisfaction.

Inclusive Care, Safe Environment

A truly affirming provider offers more than just surgical skill; they create an environment where you feel respected and safe. This means your surgeon and their team recognize that some patients may have past experiences with medical discrimination. They will take extra steps to ensure you feel in control, comfortable, and fully informed throughout the process.

At Align Surgical, we emphasize affirming care tailored to trans, nonbinary, and gender-diverse individuals. Our values include listening to your needs, using inclusive language, and giving you space to express your goals.

Reviewing Before/After Photos and Patient Testimonials

Before you choose a surgeon, take time to review before-and-after photos of their previous gender-affirming procedures. This helps you understand what results they typically create and whether their aesthetic aligns with yours.

Also, look for patient testimonials or reviews. These can give you insight into how others felt about their experiences, from the consultation to recovery. A clinic like Align Surgical shares patient stories and feedback that show its commitment to compassionate, gender-affirming care.

FINAL THOUGHTS

If you’re thinking about gender-affirming breast augmentation, know that you’re not alone, and this step can be a powerful way to feel more comfortable and at home in your body. It’s okay to take your time, ask questions, and explore what feels right for you. Your journey is yours, and it deserves care, respect, and celebration.

Whether you’re transfeminine, nonbinary, or anywhere across the gender spectrum, your reasons for considering surgery are valid. Some people want to feel more connected to their gender expression, while others seek relief from gender dysphoria. Whatever your reasons are, they are real and important.

Every transition path is different. Some people choose surgery, some don’t, and all choices are valid. There’s no one “right” way to transition. The most important thing is that you feel seen, supported, and affirmed in your decisions.

If you’re ready to learn more or talk to someone about your options, the team at Align Surgical Associates is here to help. With an emphasis on compassionate care, we are 100% dedicated to gender-affirming surgery for the transgender, nonbinary and gender-diverse community.
